Deen Dayal Divyang Rehabilitation Scheme under the Social Welfare Department of the Government of Assam 2020-21

The application for the financial year 2020-21 under the Deen Dayal Divyang Rehabilitation Scheme under the Social Welfare Department of the Government of Assam is being sought from the Divyang person. Divyang under this scheme | 20,000 (twenty thousand) one time grant for self-employment of an individual every year. Is forwarded as. Interested persons with disabilities should submit the application forms to the concerned District Social Welfare Department by August 21, 2020.

Applications can be collected from the concerned Social Welfare Department office. Applications should be examined in District Social Welfare and prepared in hard & soft copy in MS Excel and sent to the Directorate of Social Welfare by 31st August, 2020.
Requirements for Application:
(1) The age of the applicant should be between 18 to 50 years. (Attach the certificate).
(2) The applicant should have 40% or more physical disability. (Physical Disability Certificate / UDID Card sent by Health Department).
(3) Must be a permanent resident of Assam. (Inclosed the certificate)
(4) At least I have to pass 10th class.
(5) The annual income of the applicant or the family shall not exceed Rs. 2 (two) lakhs.
(6) Preference will be given to applicants trained in skills development from any recognized program. In addition, those who are engaged in business-trade / agriculture / manufacturing activities but do not have the required funds for self-reliance can also apply. (Certificate of training / Photos of own business establishment etc.)
(7) One passport size photo should be attached with the application form.
(8) The applicant should attach a proposal scheme signed by the concerned department with the application.
(9) The applicant should submit the application form along with the specific IFSC code of his bank account.
(10) The application of the beneficiary under this scheme in advance will be rejected.

ইয়াৰ দ্বাৰা অসম চৰকাৰৰ সমাজ কল্যাণ বিভাগৰ অন্তৰ্গত দীন দয়াল দিব্যাংগ পুনঃ সংস্থাপন আঁচনিৰ অধীনত ২০২০-২১ চনৰ বিত্তীয় বৰ্ষৰ বাবে দিব্যাংগ ব্যক্তিৰ পৰা আবেদন পত্র বিচৰা হৈছে । এই আঁচনিৰ অধীনত দিব্যাংগ | | ব্যক্তিৰ স্ব - নিয়ােজনৰ বাবে প্রত্যেক বছৰে ২০,০০০ / -টকা ( বিশ হাজাৰ ) এককালীন সাহায্য ( one time grant ) | হিচাপে আগবঢ়োৱা হয় । ইচ্ছুক দিব্যাংগ ব্যক্তিসকলে আবেদন পত্ৰসমূহ সংশ্লিষ্ট জিলা সমাজ কল্যাণ বিভাগৰ কাৰ্যালয়ত অহা ইং ২১ আগষ্ট , ২০২০ তাৰিখৰ ভিতৰত দাখিল কৰিব লাগিব । আবেদনপত্র সংশ্লিষ্ট সমাজ কল্যাণ বিভাগৰ কাৰ্যালয়ৰ পৰা সংগ্ৰহ কৰিব পাৰিব । আবেদন পত্রসমূহ জিলা সমাজ কল্যাণ বিষয়াই পৰীক্ষা কৰি তালিকাসহ ( Hard & Soft copy in MS Excel ) ত প্রস্তুত কৰি ইং ৩১ আগষ্ট , ২০২০ তাৰিখৰ ভিতৰত সমাজ কল্যাণ সঞ্চালকালয়লৈ প্ৰেৰণ কৰিব লাগিব ।
( ১ ) আবেদনকাৰীৰ বয়স ১৮ বছৰৰ পৰা ৫০ বছৰৰ ভিতৰত হ'ব লাগিব । ( লগত প্রমাণপত্র গাঁঠি দিব )
( ২ ) আবেদনকাৰী ৪০ % বা ততােধিক শাৰিৰীক প্রতিবন্ধকতা থকা হ'ব লাগিব । ( স্বাস্থ্য বিভাগৰ দ্বাৰা প্ৰেৰণ কৰা শাৰিৰীক অক্ষমতাৰ প্রমাণপত্র / UDID Card গাঁঠি দিব )|
( ৩ ) অসমৰ স্থায়ী বাসিন্দা হব লাগিব । ( প্রমাণপত্র গাঁঠি দিব )
( ৪ ) অতি কমেও দশম শ্রেণী উত্তীর্ণ হ'ব লাগিব ।
( ৫ ) আবেদনকাৰী বা পৰিয়ালৰ বার্ষিক উপার্জন ২ ( দুই ) লাখ টকাৰ বেছি হ'ব নালাগিব । |
( ৬ ) কোনাে স্বীকৃতিপ্রাপ্ত অনুষ্ঠানৰ পৰা দক্ষতা বিকাশ প্রশিক্ষণপ্রাপ্ত আবেদনকাৰীক অগ্রাধিকাৰ দিয়া হ'ব । তাৰােপৰি যিসকলে আত্মসংস্থাপনৰ বাবে ব্যৱসায় - বাণিজ্য / কৃষিকর্ম / উৎপাদনমুখী কাম কৰি আছে কিন্তু তাৰ বাবে প্রয়ােজনীয় অর্থৰ অভাৱত আছে , তেনে ব্যক্তিও আবেদন কৰিব পাৰিব ( প্রশিক্ষণ গ্রহণ কৰা প্রমাণপত্র / নিজৰ ব্যৱসায় প্রতিষ্ঠান আদিৰ ফটো গাঁঠি দিব ) ।
( ৭ ) আবেদনপত্ৰৰ লগত একপি পাছপৰ্ট আকাৰৰ ফটো গাঁঠি দিব লাগিব ।
( ৮ ) আবেদনকাৰীয়ে সংশ্লিষ্ট বিভাগৰ বিষয়াৰ দ্বাৰা স্বাক্ষৰিত এটা প্রকল্প ( Proposal scheme ) আবেদনৰ লগত গাঁঠি দিব লাগিব ।
( ৯ ) আবেদনকাৰীয়ে নিজৰ বেংক একাউণ্টৰ সবিশেষ IFSC কোডসহ আবেদনপত্র দাখিল কৰিব লাগিব ।
( ১০ ) আগতে এই আঁচনিৰ অধীনত সুবিধা লাভ কৰা ব্যক্তিৰ আবেদনপত্র নাকচ কৰা হ'ব ।
